or breezy nights.
Any other comfortable casual wear.
Please
use soft collapsible luggage if possible, your cabin will be more comfortable
if you are able to store your soft luggage in your personal wardrobe.

DIVING EQUIPMENT: |
Mask, Fins, Snorkel, Air
and/or dedicated Nitrox regulator with octopus and SPG/Computer, Buoyancy compensator,
Jacket, exposure suit, Compass, Knife, Dive Watch, Dive light plus back up with spare batteries, Personal photographic/video equipment with strobes and/or lighting systems.

TRIP
CANCELLATION INSURANCE |
When
booking your live-aboard dive-vacation, we recommend that you
purchase comprehensive accident, medical, baggage and trip
cancellation/ insurance
in order to protect your financial interests in the event that unforeseen
circumstances prevent the vessel from making it's scheduled trip. Please
note that should the Captain deem it necessary to cancel or to interrupt a
trip due to inclement weather or circumstances beyond Hooked on Diving's
control, no credit
or refund will be issued.
 | DIVING INSURANCE |
All who intend to participate in diving activities during their respective
live-aboard vacations must be insured with "Diving Accident
Network". (DAN), PADI Insurance or the equivalent. DAN America membership and dive
accident insurance are available to residents of the United States,
Canada, Mexico, and 44 other countries in Central and South America
and the Caribbean. You may purchase your insurance from Hooked on
Diving or you may make arrangement directly with DAN
at (800) 446-2671.

MEDICAL CLEARANCE |
It is your responsibility
in determining your medical and physical fitness to dive or to
take part in any other activities during this cruise and/or land based
activities. All activities are undertaken at your own risk without any
assurance from Hooked on Diving. If you have any questions concerning your medical or physical
fitness to dive or your ability take part in any such activities, please consult your
physician.
 | CLIENT / EQUIPMENT LIST |
A complete client list detailing flight information for
each passenger as well as any special dietary requests must be dated, signed received and confirmed at Hooked on
Diving 60 days prior to departure. If you intend to rent any diving equipment or
any other special equipment from us, we encourage you to let us know in
advance as there is a limited number of rental pieces on board at any given
time. All rental items are on a first come, first reserved policy, therefore
it is possible that we could run out of the particular piece of equipment
that you need if we do not know in advance.
